Implants
Implant dentistry continues to evolve to meet the patient's demands for aesthetic tooth replacements with minimal downtime and inconvenience. This technique allows replacing the missing natural teeth by their artificial analogues.
An implant is a titanium-made screw which is inserted into the crestal bone.

Tooth implants consist of two major parts: the implant itself which nothing but an artificial root and of the abutment. Abutment is connecting element used to attach the dentures to dental implant that is integrated into the bone. These abutments can be made from a variety of materials, such as titanium, surgical stainless steel and gold. More modern abutments can now also made from zirconia, which is a white ceramic and which better complement the esthetics of a dental implant restoration. The two images to the right show a ceramic abutment and the ceramic crown bonded to it.

The dental implants are fused into the bone and the healing or integration process occurs during 3-4 months or even 6 months. Dental implant success is related to operator skill, quality and quantity of the bone available at the site, and also to the patient's oral hygiene. The general consensus of opinion is that implants carry a success rate of around 95%.

Advantages of the implanted teeth.
The dental implants successfully replace the teeth lost as a result of trauma or disease.
1. With implantation the adjacent teeth are not drilled and are left intact.
2. As a result of the teeth restoration the bridge-like denture can be mounted on the implants.
3. If the patient lacks the tooth or teeth completely it helps to avoid the detachable dentures and to have a much more aesthetic and comfortable solution and give the long term benefits as opposed to the bridge treatment..
4. Implants can be the basis for the better fixation of the removable dentures with much increased power of fixation.
5. Implants prevent the bone loss or reduction due to prior loss of teeth and absence of the mechanical stress of chewing.
Procedure

Typically implantation occurs in three basic steps.
The first step includes the insertion of the dental implant into the jawbone and occurs under Implant surgery is performed as an outpatient under general anesthesia (if several implants are to be placed) or with local anesthesia (for simple cases) by trained and certified. A single implant procedure that involves an incision and "flapping" of the gum or gingiva (to expose the jawbone) takes about an hour, sometimes longer; multiple implants can be installed in a single surgical session lasting several hours. A cover screw is placed on the implant, then the gingiva or gum is sutured over the site and allowed to heal for several months for osseointegration.

The second step is to open the soft tissues and to insert the interim abutment and the temporary crown. This the gum grows in the right scalloped shape to approximate a natural tooth's gums and allows assessment of the final aesthetics of the restored tooth. Once this has occurred a permanent crown will be fabricated and placed on the implant. The fabrication of the permanent crowns and dentures is the final, third step of the procedure.

The dentist will then determine what type of prosthesis will be fabricated.
Single implant.
The single implant allows to replace the missing tooth without affecting the adjacent teeth. The neighboring teeth will not be drilled.
Implants as basis for detachable dentures.
Several implanted teeth will firmly hold the new detachable denture in case of absence of the complete teeth row. For the lower jaw one needs 2 implants, for the upper side 4 implants will be enough.
The lower jaw.
For the normal distribution of the prothesis weight on the gum of the lower jaw 2 implants with a length of 7 mm would be enough.

The upper jaw.
To adjust the prothesis on the upper jaw 4 inserted implants would be enough as the upper jaw is less strong than the lower one. 4 implants would be a good basis for the denture which will not lie on the palate as the case with the usual detachable dentures. The detachable upper denture will be held in place by implants with the length of the collar height of 10 mm and the platform diameter of 4 mm, evenly inserted in the forward part of the jaw. The denture is then fixed with the beam structure. The healing ball abutment is then placed onto the implant. The future denture beam is attached with the help of the beam structures.

Multiple implants.
One can fully restore the missing teeth in the upper and lower jaws, the amount of implants is always less than the amount of the teeth restored. For instance, to replace the four missing teeth three implants would be enough.
The bridges and the implants.
The implants can be the basis for the usual bridge-like denture. After the implants are in place and healed the dental bridge can be manufactured and inserted in the traditional manner.
